At DayBreak, we are dedicated to ensuring that elders in our community can live independently and with dignity. Our Rapid Response Case Management program provide comprehensive support to address the urgent and ongoing needs of our clients.

Our Services Include:

  • Access to Care: Helping clients connect with primary care physicians, Medicare, and Medi-Cal healthcare services.

  • Mental Health Referrals: Providing referrals to mental health professionals to support emotional and psychological well-being.

  • Appointment Monitoring: Ensuring clients attend their medical and social service appointments.

  • In-Home Support Resources: Facilitating access to resources that allow clients to receive IHSS or care in their homes.

  • Housing Navigation: Assisting clients in finding and securing safe and stable housing.

  • Access to Food: Connecting clients with food resources to ensure they have access to adequate nutrition.

  • Social Security and Supplemental Security Income: Helping clients navigate the complexities of Social Security and SSI benefits.

  • Replacement of Identification Documents: Assisting clients in obtaining necessary identification documents.
